When Survivor appeared on CBS for the first time, it quickly became a topic of conversation around water coolers everywhere. Even people who didn't watch the show couldn't escape conversations about which castaway would be voted off the island next. Today, there are dozens of reality shows that offer to turn everyday people into chefs, actors, singing stars, brides or grooms, millionaires, and almost anything else you can imagine.

And while people make the obligatory negative remarks about reality TV, many harbor secret addictions to one show or another. What makes reality TV so popular? There are probably several factors.

First, reality TV participants come from all walks of life. Whether you're gay or straight, male or female, Christian or Buddhist, Black or white, it is easy to turn on reality TV and spot someone like yourself whisked away into an adventurous or luxurious lifestyle. We experience a vicarious thrill seeing ordinary people like us compete for fabulous prizes. And, if you are being absolutely honest, you have to admit that you've sometimes thought, "I could do that!"

Another reason people enjoy reality TV is the suspense. On most shows, each episode finds another participant being eliminated until there is only one player left standing. We agonize with our favorite players as they endure challenges and hardships, hold our breaths when it looks like they may be on the chopping block, and breathe a sigh of relief when they squeak by for one more week.

Watching reality TV also gives us an avenue to communicate with acquaintances and co-workers. As the second season of American Idol drew to a close, for instance, you could start a conversation with a complete stranger simply by asking, "So, is it going to be Clay or Ruben?"

Still another reason reality TV is popular may have to do with certain socioeconomic factors. With so many people in debt, the cost of living consistently outstripping people's paychecks, relatively high unemployment, and wars being fought in Iraq and Afghanistan, reality TV gives people something less threatening to focus on. It may even help people feel better about their own lives. "I may have it bad," some viewers think, "but at least I'm not wearing filthy clothes and eating insects." Or, "I may have had a bad day of work, but at least the entire viewing public isn't watching Simon Cowell blast my singing voice."

Finally, reality TV initially had a strong viewing because it was something unique. Audiences in the United States were used to watching television actors deliver scripted lines-we were completely unprepared for a show without a script featuring real people engaged in real competition. The real live cat fights, backbiting, and scheming delighted wide eyed viewers.

Of course, from the beginning there were a few dissenting voices claiming that reality TV was somewhat less than real. Furthermore, familiarity has begun to breed contempt. The second installment of Survivor, for instance, was the most watched series of the 2000-2001 TV series. Survivor China, the fifteenth installment in the Survivor series, is ranked number eleven among the current fall lineup. Other reality TV shows have seen similar rating drops as their novelty faded.

But even if reality TV is no longer quite as robust as it once was, it is far from dead. New reality TV concepts are pitched every season, and millions of fans continue to tune in to watch their favorite reality programs. For all the reasons discussed above, reality TV is definitely here to stay.

Polycystic Ovary Syndrome (PCOS)

The endocrine disorders among females are of various types but the most common one is the polycystic ovary syndrome (PCOS) which is basically an inherited disorder and can be received from either parent. The chances of occurrence of this disorder vary from 5-10% among the females of age group of 12-45 resulting in female sub-fertility. This endocrine disorder can be identified by anovulation which is diagnosed by irregular menstruation, amenorrhea, polycystic ovaries, ovulation-related infertility, excessive secretion of androgenic hormones that cause hirsutism and acne. High cholesterol level, type 2 diabetes, insulin resistance are other known symptoms. All these symptoms vary among different individuals. The disorder is known by a number of other names like polycystic ovary disease, functional ovarian hyperandrogenism, ovarian hyperthecosis and Stein-Leventhal syndrome. A polycystic ovary has abnormal number of eggs that can be viewed near its surface resembling cysts.

Polycystic ovary syndrome is generally described by two definitions. First definition was given by NIH or NICHD in 1990 which suggests that if a female is suffering from oligoovulation, shows signs of androgen excess and other entities that result in polycystic ovaries then the female is suffering from this endocrine disorder. The second definition was given in a workshop sponsored by ESHRE/ASRM held in Rotterdam in 2003 which predicts that if a female is suffering from oligoovulation or anovulation, has excess androgen activity and symptoms of polycystic ovary then she is suffering from polycystic ovary disease. The second definition appears to be wider and acceptable. The chief symptoms of the polycystic ovary syndrome include menstrual disorders chiefly amenorrhea and oligomenorrhea but other menstrual disorders may also crop up. Chronic anovulation results in infertility. High levels of androgens result in acne and hirsutism. Hypermenorrhea and other symptoms can also make their appearance. About three quarters of the females suffering from this endocrine disorder generally suffer from hyperandrogenemia. Central obesity and insulin resistance are also noticed. Serum insulin and homocysteine levels are significantly higher in females with this disease.

It is not always necessary that the women suffering from polycystic ovary syndrome (PCOS) may have polycystic ovaries and similar is the condition that all the women with polycystic ovaries may not suffer from this syndrome. The syndrome can be easily diagnosed by the pelvic ultrasound but other diagnostic tools are also available. History of the individual based on menstrual pattern, obesity, hirsutism and absence of breast development can help the medical professional. Gynecologic ultrasonography can be performed which helps in the detection of small ovarian follicles. These small follicles are believed to be formed due to disturbed ovarian function where ovulation has failed to take place due to absence of menstruation. In a normal menstrual cycle a single egg is released from the dominant follicle. After ovulation, the remnant of the follicle is converted into a characteristic structure known as corpus luteum formed by the action of progesterone. This structure finally disappears after 12-14 days. In polycystic ovary syndrome, although a number of follicles are formed but none of them grows more than 5-7 mm in length and fail to enter the preovulatory stage of the menstrual cycle. According to the Rotterdam criteria there must be 12 or more than 12 small follicles detected in the ultrasound. These small follicles are generally present near the periphery of the ovarian wall giving it the appearance of string of pearls. The ovary enlarges and attains a size which is 1.5 to 3 times greater than the normal size and this is due to the presence of these abnormal follicles.

Laparoscopic examinations depict the presence of a white, smooth outer surface of ovary. The serum (blood) levels of androgens specifically androstenedione and testosterone are elevated. The levels of dehydroepiandrosterone sulphate are also higher. The free testosterone levels are also high and it gives the best clue about the presence of this syndrome. The free androgen index of the ratio of testosterone to sex hormone binding globulin (SHBG) is generally higher but it is a poor indicator. Some blood tests are also suggested but they are not good indicators of the diagnosis of the polycystic ovary syndrome. The ratio of LH (Luteinizing Hormone) to FSH (Follicle Stimulating Hormone) is greater than 1:1 as tested on the third day of menstruation. Among the obese women the levels of the sex hormone binding globulin (SHBG) is generally low. Fasting biochemical screening and lipid profiling of the individual can be carried out while searching for this syndrome. A 2-hour oral glucose tolerance test (GTT) of the suspected individuals can be carried out which indicates impaired glucose tolerance in 15-30% patients of this syndrome. Insulin resistance is very commonly noticed in the patients of polycystic ovary syndrome. Other clinical disorders may also be associated with menstrual abnormalities namely Cushing's syndrome, hypothyroidism, congenital adrenal hyperplasia and pituitary disorders.

Polycystic ovary syndrome (PCOS) is a generically inherited condition. It is inherited in an autosomal dominant system with higher risk of occurrence in females. The chances of inheriting the gene responsible for this syndrome are 50% if the parent is carrying the gene. The gene responsible for this syndrome can although be inherited either from the father or the mother and the gene can be passed to the sons but the symptoms may arise only in the daughters. The gene responsible for this disorder has not been yet identified. Polycystic ovaries generally develop when the ovaries are stimulated to produce excessive amounts of the male hormones particularly testosterone. This may happen due to release of excessive amounts of the luteinizing hormone (LH) from the anterior pituitary gland or elevated levels of insulin in the blood of women who are sensitive to insulin or reduced levels of sex hormone binding globulin (SHBG) in blood which results in increased level of free androgens. The syndrome has acquired its name polycystic due to resemblance of the small follicles with the cysts. The follicles develop from the primordial follicles but their development ceases at the antral stage due to the disturbed ovarian function. These cysts like follicles get arranged at the periphery of the ovarian wall. Majority of the patients with this disorder in general show insulin resistance and this can cause abnormalities similar to those observed in the hypothalamic-pituitary-ovarian-axis.

The symptoms of the polycystic ovarian disease are very complex and may not be same for all the patients. In many cases it can be characterized by hyperandrogenism and insulin resistance. Majority of the cases of this disease have a genetic basis. The excessive amounts of the adipose tissue in the obese individuals also increase levels of androgens and estrogens. Adipose tissue carries an enzyme identified as aromatase that participates in the conversion of androstenedione to estrone and testosterone to estradiol. Hyperinsulinemia causes an increase in the GnRH pulse frequency, increased ovarian androgen production, decreased follicular maturation and decreased sex hormone binding globulin levels that ultimately result in polycystic ovary disease. Chronic inflammations may also result in this syndrome. A study carried out in the United Kingdom indicated that the incidence of polycystic ovary disease is higher in the lesbian women than the heterosexuals. The medications given to the patients of this disease generally focus on lowering of insulin levels, fertility restoration, hirsutism or acne treatment and prevention of endometrial hyperplasia, endometrial cancer and restoration of regular menstrual cycle. Cases where the disease is associated with obesity, weight loss is the effective strategy for the commencement of regular menstruation. Low carbohydrate diet and regular exercise may help in weight loss.

All the females with the polycystic ovary disease may not face the difficulty of becoming pregnant only those suffering from anovulation may face the problem. Patients with the problem of anovulation may be treated with clomiphene citrate and FSH injections. The patients who fail to give positive results with clomiphene and FSH treatments are treated with assisted reproductive technology procedures like controlled ovarian hyperstimulation with follicle stimulating hormone (FSH) injections followed by in vitro fertilization (IVF). Surgery is generally not performed in case of the polycystic ovary but a laparoscopic procedure known as ovarian drilling is generally carried out. Hirsutism can be treated by using an effective standard contraceptive pill. The key ingredient of the contraceptive pills is cyproterone acetate which is a progestogen. This compound is anti-androgenic in action and blocks the activity of male hormones that are responsible for acne and unwanted hair growth on face and over body. Other drugs that carry anti-androgen effects include flutamide and spironolactone that can effectively reduce hirsutism. Spironolactone is the most commonly used drug in the United States. Menstrual problems can be regulated by the use of contraceptive pills but these drugs can cause additional problems if continued for a long time. Two inositol isomers namely D-chiro-inositol and myo-inositol have given promising results against this syndrome.

Women suffering from polycystic ovary syndrome are at the risk of getting affected with endometrial hyperplasia and endometrial cancer. These clinical manifestations may crop up due to over accumulation of the uterine lining and absence of progesterone which is responsible for the prolonged stimulation of the uterine cells by estrogen. These symptoms set a positive background for the appearance of other health problems like obesity, hyperinsulinemia, hyperandrogenism, type-2 diabetes and insulin resistance. A study conducted in 2010 spotlighted that the women suffering from polycystic ovary disease are at an elevated risk of getting affected with type-2 diabetes and insulin resistance. High blood pressure, depression or depression with anxiety, miscarriage, excessive weight gain, cardiovascular disease, acanthosis nigricans, autoimmune thyroiditis are other risks associated with this syndrome.

I am not a regular listener of music but I am really inspired by the performance of the Coke Studio and this whole show. I have indirect affiliation with literature and performing art due to the fact that my grandfather Fida Mutwahir was a prominent Pashto Poet of his era. Those who belong to the soil of Puktoonkhwa must be familiar with his name and his contribution towards Pashto Literature. The unique intermingle of our folk music can be experienced at the Coke Studio. It is a mix enchantment of eastern and western music that accentuates more on folk music. Our singer's proved to be an asset in the Music Industry through out the world, not just representing Pakistani culture, customs and traditions but also spreading out the message of Peace, Love and Harmony through their music.

Coke studio is playing a vital role by promoting the performing art and providing an intercultural platform to both renowned and fresh talent to collaborate with each other by using the mix of traditional and ultra modern music instruments and technology. There stunning performances beat the drums up and conveys the message to the world that we stand united irrespective of race color, cast and creed. Our artists can play western "Guitars" with traditional "Rabab" and our singers can sing English songs with Urdu at the same time. The female guitarists and singers in the "Cook Studio Show" are evident to the fact that we believe in equality of rights and freedom of speech. We are not extremists and terrorists. We are civilized-moderate- peace loving Muslims.

Due to limited number of opportunities our youth is flooding Western world one way or the other, legally or illegally, by walk or by planes. It bleeds my heart, but when I look at the big picture, I try to calm down myself because I know they are there for a happy, prosperous and brighter future and will contribute towards the development of the country in different fields of life. The most disappointing situation is the unprofessional attitude of the western media which is not just involved in racial bigotry of Muslim's but continuously misleading the world through the false and negative propaganda particularly against Pakistani's which is truly not aligned with reality, thus elevating racism and producing hatred among the Western and American natives as they are truly media believers.

We have sacrificed our heros in war against terrorism and the recent one is once again from Khyber Pukhtoonkhwa, Siffat Ghayour (Commandent FC) who touched martyrdom to protect his soil from terrorism and extremism, Siffat Ghayour played a vital role in eliminating the lines of Taliban and created chaos amongst them, due to his successful counter insurgency strategy, he was on the hit list of Taliban. We have sacrificed hundreds of Siffat Ghayours in this war and many more will be born to rise against those who try to take our national interest in their hands.

Ever since Pakistan came into being, we find this country in hot water, we are at war with ourselves, we are at war with our neighbors, we are at war with natural calamities, we are at war with the Taliban, we are at war with target killings, and our economy is dwindling and shrinking day by day. The recent wave of violence in Karachi is on top of everything, the country is already in volatile situation and these recent incidents will complex the situation even further. Let's not forget that Karachi is the main hub of economic activities and the situation of Karachi reflects the country's economic state and affairs. Crises in this city is often seen as an indicator of the situation in the country as a whole.

A flood ravaged hundreds of villages in Khyber Puktoonkhwa and Punjab and killed more then 1500 people and has already entered Sindh. People are being forced to evacuate the villages; unfortunately we are always caught outrageously by disasters. "Plan ahead, it wasn't raining when Noah built the ark" Whoever sad it, sad it for this day. Expect the unexpected from life and nature. Tough times do come in the history of the nations but those who are ready and prepared for it, face it courageously. No contingency plan is in place in order to deal with natural calamities. Our leaders can import luxury cars but they can not buy boats, ambulances, fire brigade trucks, helicopters and life saving equipment particularly for emergencies. Government is allocating hundreds and hundreds of acres for housing schemes however; there are no permanent compounds for those affected by the floods or those who become IDP's. And one must say thanks to God for testing us in summers, the disasters mostly blew our doors during summer vacations when schools are unoccupied and free to utilize as shelter houses.

Contingency plan should be in place irrespective of nature of the disaster weather its earthquake, flood, bombing or whatever. The role of existing disaster management cell is limited to statistics and distribution of donations and it bears a very bad reputation due to its poor aid management. One can still find the earthquake relief goods in the markets such as blankets and tents, sold in the open market. Since then people hardly trust these government institutions for the distribution of donations Same is the case with international community, their response towards this recent disaster was quite slow and we did not get enough aid from the international community although I personally feel that we should not rely on foreign aid but even if we don't need it, our leaders are used to publicize and exploit such occasions and disasters in the larger interest of their Swiss bank accounts.

In the wake of recent scenario I have come to this conclusion that we should form committees on grass root level, at Mohallah, Village, Tehsil and District level. The collection points should be the Mosques of every Mohallah. In every one KM area there are 5 to 8 mosques and every mosques serves at least 30 to 40 houses in each area. Religious leaders and scholars should come forward; go door to door and collect donations and should take responsibility for distribution of donations as well. (Wake up Molvi Saabs, you have to be on your toes the way you are active on the occasion of Eid-Ul-Adha, searching for Qurbani ka Bakra's door to door). Turn on the loudspeakers of your mosques; while you preach do not remind us of our wrong doing. Yes I know it is because of our wrong doings that we are struck by floods and earthquakes but let bygone be bygone or we can discuss this later. It is time to act. Please somebody has to translate this to our Molvi Sahiban. And one more important thing, Ramadan is ahead so please no fights over moon sighting for God sake. Similarly, Dr Sahiban are also requested to collect medicines for the epidemic caused by the floods from the pharmaceutical companies who have sucked the blood of this nation, making millions of profit every year, sending troops of troops abroad for conferences, seminars and arranging international family trips for their partners in crime (with apology from those we have not been on these trips ever, good guys are excluded and I am asking the good ones to collect donations although if bad ones wants to join, they are not banned).

The estimated total population of our country in 2010 is 170200500 and we are the sixth most populous country in the world. The number of people directly affected by the flood is 8000000. The remaining population is 169400500. It is a challenge and battle for those remaining 169400500 people to win the hearts and minds of these 8000000 flood victims. There are very less number of opportunities available in this country and of course we do lack leadership but let me assure one thing to all of you, this country does not lack resources and talent; this nation has enough spirit and compassion to deal with wars and disasters which have already been observed during the earthquake.

It gives me immense pleasure when my eyes sight these young kids standing in the scorching heat in every nock and corner of the country, carrying buckets, running after cars and buses risking their life for their loved ones and collecting donations for them. I am not hopeless from the youth of this country and I have faith in my youngsters, they aim high and there vision is beyond the sky. They have achieved high and they will achieve higher still inshallah. Our youth hate corruption and they are against dowry and bribery. We want leaders with genuine degrees, honest and fair reputation and sound professional attitude and character. Let the world honor us with medals not shoes. We want to stand among the civilized nations and we will rise against all the odds. Let us support our youth and encourage them that they are not alone in this noble cause. Give them what they want, from donation to support, from books to guitar and career to leadership, give them opportunity and let them be the true leaders and messengers of this soil, remember what is done to children today, will they return to the society tomorrow. Donate generously for the well being and rehabilitation of the flood victims, donate it the way you want, as much as you can and in whatever way you can, I have no concern with that as long as you are donating.

Production of glass and glass bead making in the Czech Republic came back from early Bronze Age, when the region was colonized by the Celts, who possessed the ability of glass and enamel. Not only did the Celts in making and processing of glass beads were involved, but so were the Benedictine monks in the monasteries. A glass mosaic titled "Judgement Day", the golden door of the Cathedral of St. Vitus in Prague adorns is a testament to the skill of the Middle Agesglass-makers.

Czech glass can be found in Corinthian excavations dating back to the 11th and 12th centuries. During this time, the art of glass making ceased to the solely the milieu of Monks and it moved out into the forests where many cottage glass-works arose. Vast quantities of wood were needed to fuel the furnaces, so the densest forests became the centers for this early art form.

The forest glass-works created "forest glass" of a greenish color created by imperfect refining of raw the materials potash and quartz sand--the main ingredients in glass.

In 1588, Emperor Rudolf the 2nd invited Mr. Ottavio Miseroni, the first person to perfect the art of cutting and shaping diamonds, to Prague. Miseroni opened a workshop for cutting diamonds. With his diamond cutters, he discovered a way to decorate glass by using stone or copper wheels. Engraving was born. The most important craftsman in Prague to come from Miseroni's workshop was Caspar Lehmann, who became an Imperial Court engraver. Rudolf the 2nd became a generous sponsor and Prague became the center of this new glass craft. From Prague, the craft spread to all of Europe.

During the 1600s, the popularity of engraved glass continued to increase and by the end of the 17th Century, Czech glass achieved world wide recognition. Particularly fire-polished beads, have been produced in the area of the Czech Republic for over 200 years. This craft started as a cottage industry with the forest glass makers, but today the art of crafting Czech faceted beads is done in a factory.

They are such a huge part of Czech industry that the Soviet Union created a state agency called Jablonex to distribute the beads. The breakup of the USSR saw a resurgence of onesproduced in cottage industries. Today, there are many companies that export Czech facetted beads to over 80 countries. The small enterprises are greatly assisted by the fact that Jablonex is no longer taking on any new United States clients.

The most common shape for the Czech bead is round, although the fire polished beads are more oblong. They are available in other shapes, too: bicone, rondelle, roller, roundlet, pear and parachute. The fire polished ones come in a variety of colors. The richness of color is one of the earmarks of Czech beads.

Most people think all fire polished beads come from the Czech Republic. In the last ten years or so, India has begun producing them too. Fire polished beads are distinguishable by there soft rounded edges on the face.
